Laser and Refractive Surgery

Refractive ophthalmic surgery allows to correct refractive errors permanently in a safe, effective, and reliable way with very few risks of complications.

With around more than 25 million refractive laser surgeries performed to date, laser in-situ keratomileusis (LASIK) is the most commonly used refractive surgical procedure worldwide. Alternatives to LASIK include surface ablation procedures (PRK, trans-PRK, Femto-LASIK) and intraocular lens implantation.

If ocular accommodation is lost as in age of more than 40 years, removal of the natural crystalline lens and implantation of modern multifocal intraocular lenses (refractive lens exchange) simple or toric IOL, provide an alternative means of correcting myopia, hyperopia and presbyopia.

PRK and Trans-PRK

PRK surgery is performed to correct mild to severe myopia, mild to moderate hyperopia, and astigmatism. In PRK the outermost layer of the cornea is shaved off and then the laser is applied. In trans-PRK laser is directly applied to the corneal epithelium. It is more advanced than PRK.

Advantages

  • In PRK surgery no corneal flap is created.
  • It is recommended for patients with thin cornea or for people who are engaged in activities that put them at risk for eye injuries after surgery.

Disadvantages

  • The visual recovery period is longer than LASIK surgery usually 60-80% in the first 3 days and 100% in the next 2 weeks.
  • The patient can feel ocular pain and more discomfort as compared to LASIK surgery on the first night.

Results are comparable to LASIK surgery.

LASIK and FemtoLasik

In LASIK Surgery, very thin corneal flaps are created accurately using an ultra-fine blade (Lasik) OR with an ultra-fast femtosecond laser (femto-Lasik). After the flap has been created the surgeon lifts it like a book cover with a specially designed instrument. The Excimer laser is then used to reshape the cornea to correct refractive errors. At the end of the procedure, the flap is replaced in its original position.

Advantages

  • Less pain and discomfort is experienced at first night.
  • Visual recovery time is 24-36 hrs.

Disadvantages

  • Flap related complications (uncommon) can occur later in life.

Results are comparable to LASIK surgery.

LASER is NOT Recommended In

  1. People younger than 18.
  2. People whose refractive error has not stabilized for at least 06 months prior to surgery.
  3. People who have unrealistic expectations about the surgical outcome.
  4. People with uncontrolled diabetes.
  5. People with a history of corneal herpes simplex, keratoconus, and corneal scarring.
  6. People already have glaucoma or cataract.
  7. Pregnant or breastfeeding women.

POST OP CARE:

  1. Your first post-operative visit should be scheduled as per the advice of your surgeon.
  2. At your first post-operative visit, your surgeon will test your vision, and examine your eyes.
  3. Avoid wearing contact lenses for at least the first three months.
  4. Avoid wearing eye makeup and applying lotions or creams around your eyes for at least the first three days or till your surgeon gives you permission.
  5. Avoid certain contact sports, such as boxing, football, karate, etc. for at least 10 weeks after Lasik.
  6. Avoid washing your eyes with water for 7 days.
  7. Avoid rubbing and scratching your eyes in the first few weeks after surgery.
  8. The bandage contact lens will be removed by your surgeon in OPD after 5-7 days in case of PRK or TransPRK.
  9. In the first few months after surgery, you may experience minor fluctuations in your quality of vision. Complete recovery and stabilizing your vision may take 3-6 months, during this period you may also experience some complications such as night driving difficulties, and seeing halos or glare in your field of vision.
